To verify your identity, the Ministry of Health requires you to submit a copy of government-issued photo ID and proof of eligibility for public health funding. You will need this with your form.

New Zealand Passport

New Zealand Driver’s License

New Zealand Passport

Birth Certificate

New Zealand Visa (must be more than 2 years/resident visa)
